How to use this agreement
This service agreement template helps set clear expectations, communicate roles and responsibilities, and support a professional working relationship.
- Fill in the relevant details and customise the terms.
- Review the agreement with the participant before signing.
- Keep a signed copy for your records.
- Ensure the terms suit the supports, funding arrangements and current requirements.
Before using the original PDF: It includes a fixed 24-hour cancellation example and a service-suspension clause. These are not universal NDIS rules. Review and adapt them before use, taking account of the current pricing arrangements, participant circumstances and appropriate advice.

Complete these details in your downloaded copy, not on this website.
1. Purpose of this agreement
This agreement outlines the services provided, responsibilities of both parties, and payment terms to ensure a clear and professional working relationship.
2. Services to be provided
The support worker agrees to provide the selected services:
- Personal care.
- Social and community access.
- Daily living support.
- Life skills development.
- Transport assistance.
- Other agreed services.
Record the relevant NDIS line items, days and times of support, and support locations. Describe a specific schedule where appropriate rather than leaving the arrangement unclear.
3. Rights & responsibilities
Support worker responsibilities
- Provide services in a safe, respectful and professional manner.
- Adhere to the NDIS Code of Conduct and relevant legal requirements.
- Maintain confidentiality and respect the participant’s privacy and dignity.
- Keep accurate records of support provided.
- Give adequate notice of changes to agreed services.
Participant responsibilities
- Respect the support worker’s time, policies and boundaries.
- Provide relevant information about support needs and health conditions.
- Notify the provider in advance when services need to be cancelled or rescheduled.
- Discuss how to maintain a safe and appropriate environment for support.

4. Payment & invoicing
Agree and record:
| Detail | Complete in your agreement |
|---|---|
| Hourly rate | $_____ per hour |
| Permitted travel costs | Agreed amount and basis |
| Invoicing schedule | Weekly / fortnightly / monthly |
| Payment due | Within _____ days of invoice issue |
| Payment method | Bank transfer / plan manager / other |
| Cancellation policy | Agreed terms consistent with applicable requirements |
| Late payment process | Reminder and follow-up arrangements |
The original cancellation example states a full session fee for less than 24 hours’ notice or a no-show, and no charge for more than 24 hours’ notice. Do not copy this automatically. Confirm whether a charge is permitted, and the applicable notice period and conditions, before agreeing the policy.
5. Agreement duration & review
Record the start date and end date, or when the agreement will be reviewed or updated. The template offers review intervals of three months, six months or annually.
6. Termination of agreement
Record the agreed written notice period and appropriate grounds and procedures for ending services. Discuss arrangements for any transition of support.
The original template includes failure to meet responsibilities and breach of the Code of Conduct or misconduct as examples. Adapt this section to the actual service arrangement.
7. Confidentiality & privacy
Both parties agree to maintain confidentiality of personal and sensitive information. Records should be securely stored and used for appropriate professional purposes.
Explain when information may be shared with consent and any circumstances where disclosure is required or permitted by law. The original PDF’s consent wording should be reviewed to cover these circumstances accurately.
8. Dispute resolution
Discuss disagreements respectfully and explain the options for resolving them. The original template suggests involving an appropriate third party if needed. Include clear complaint contacts and avoid implying that a participant must use mediation before raising an external concern.
9. Signatures & agreement acceptance
Both parties should understand the agreed terms before signing. Record the support worker’s signature and date, and the participant’s or authorised representative’s signature and date. Give both parties a copy.
