How to use this report
This guided template helps you summarise support and participant progress over a period of time. Read the prompts before preparing a report for the participant or an appropriate recipient involved in their support.
This is a participant progress report, rather than a template for daily session notes. Use your regular session records to help prepare it.
Participant details
The template asks for:
- Name and relevant identifying details.
- NDIS number and date of birth, where needed.
- Service agreement dates.
- Support worker name.
- Services provided.
Complete and store the report in your own secure system. Include only relevant information and share it with appropriate authority.
NDIS participant goals
List the participant’s current goals from their plan. These are the goals you have been supporting them to work towards. The supplied template provides room to identify four goals.
Support approach
Describe how you have been supporting these goals. This might include routines, strategies, co-designed activities or specific challenges you have helped with.
Example from the guide: “We’ve focused on building confidence and consistency in the kitchen through weekly meal planning and cooking sessions. We’ve also been supporting independent travel by gradually introducing bus routes together.”
Progress towards each goal
Repeat this structure for every relevant goal:
Goal
Copy the participant’s goal here.
Progress summary
What has been achieved so far? Have there been changes in confidence, independence, engagement or daily function?
Examples & evidence
Include activities, milestones or observations that demonstrate progress. Be specific and factual, and distinguish your observations from information reported by someone else.
The template includes three detailed goal sections. Add another section if reporting against a fourth goal.
Participant voice
Let the participant speak for themselves, or describe how they have expressed feedback. This could be a quote, artwork, a sentence they have written or a relevant story.
Example from the guide: “I like going on the bus now, I know where to get off and can get around on my own.”
Support worker reflections & recommendations
What is working well?
Where have you seen the most progress or positive engagement?
Areas for future focus
What could you build on? Are there new goals the participant is ready to explore?
Other recommended supports
Note other supports that may benefit the participant, and why, within your role and competence.
Additional comments
Record relevant barriers, strategies trialled and information that may help people involved in the participant’s support.
